According to the Malaysian Society of Aesthetic Plastic Surgery (MSAPS) 2023 report:
- Malaysia recorded over 320,000 non-surgical aesthetic procedures (filler injections, Botox, chemical peels, skin tightening, etc.)
- Nearly 47,000 surgical cosmetic procedures were performed—a rise of 18% compared to the previous year.
- Shockingly, more than 60% of non-surgical procedures were done in facilities without any licensed doctor supervision.
I have personally treated severe complications from home-administered fillers, illegal silicone nose jobs, and even cases of sepsis following liposuction at unlicensed "underground" spas. Some patients lost their eyesight. Some nearly lost their lives.
Case 1:
A young woman received a lip filler injection from an uncertified “beauty technician.” Her lips quickly turned blue and became excruciatingly painful. At the hospital, I diagnosed a vascular occlusion—the blood vessels were blocked, and tissue was starting to necrotize. Though we intervened, the damage—both physical and psychological—was permanent.
Case 2:
A woman in her 40s underwent abdominal liposuction at a so-called “premium aesthetic center” on the outskirts of Kuala Lumpur. There was no sterile equipment, no anesthesiologist, and no postoperative care. Two days later, she was admitted to the hospital in septic shock and almost died.
- Lack of awareness about medical risks: Many people think aesthetics is “beauty,” not medicine. They don’t realize that even a simple injection can cause stroke, vascular blockage, or death if done incorrectly or in unsterile conditions.
- Overtrust in cheap services and flashy marketing: Social media is flooded with unregulated, low-cost aesthetic offers. Users are lured in by dramatic before-after images without verifying the practitioner’s credentials.
- Loose legal oversight: There is still no strict enforcement or licensing for non-invasive aesthetic procedures in many areas.
What I constantly emphasize: Any aesthetic procedure—whether it's filler or liposuction—is a medical procedure. It must be done by certified doctors in sterile environments with emergency support available.
Did you know that botox or filler injected into the wrong place can block arteries to the brain or heart, leading to stroke or heart attack?
Many aesthetic patients are not screened for underlying heart conditions, such as:
- Hypertension
- Irregular heart rhythms (arrhythmias)
- Heart valve disorders or a history of ischemic heart disease
Without proper screening and without an anesthesiologist present, these patients are at high risk of cardiac arrest during or after procedures. This is why I always require full pre-op health screening, including cardiovascular tests, for every single patient.
- Choose a doctor certified in Plastic & Aesthetic Surgery
- Ensure the clinic has official medical licensing (LCP – Letter of Credentialing & Privileging)
- Undergo full health screening before any procedure—even for basic injections
- Don’t chase cheap prices—chase safety
Everyone has the right to feel beautiful—it’s a personal choice. But no one should pay for beauty with their blood or health.
As a doctor, I don’t reject the aesthetic industry. In fact, I love it—because it can bring confidence and happiness. But only when aesthetic enhancement is guided by medical ethics and safety standards can we truly call it the art of healing.
